For that reason, claims such as “a certain percentage of all cats have dental disease by a specific age” should not be treated as an absolute rule. Can a Cat Have Gingivitis and Still Eat?
Yes. A cat can have gingivitis and continue eating normally or nearly normally. Eating does not prove that a cat’s mouth is pain-free.
Cats may compensate for oral discomfort by changing how they chew, swallowing food with less chewing, favoring one side of the mouth, or choosing foods that are easier to eat. These changes can be subtle enough that owners do not immediately recognize them.
This is why regular oral examinations are important. Do not wait for a cat to stop eating before taking red, swollen, bleeding, or otherwise abnormal gums seriously. A veterinarian can determine whether the inflammation is gingivitis or part of another dental or oral condition.

What Causes Gingivitis in Cats?
Gingivitis in cats develops when the gum tissue becomes inflamed. Plaque-associated inflammation is a major cause, but not every case of red or inflamed gums has exactly the same underlying cause.

Importantly, plaque can begin accumulating even when a cat’s teeth still appear relatively clean. Therefore, visible tooth color alone is not a reliable measure of periodontal health.
Calculus and Tartar
Plaque and tartar are not the same thing.
Plaque is the soft bacterial biofilm that develops on teeth. Over time, minerals in saliva can cause plaque to mineralize into a hard deposit called dental calculus, commonly known as tartar.

Individual Susceptibility
Two cats with similar amounts of visible plaque do not necessarily develop identical levels of gingivitis.
The severity of inflammation depends partly on the individual cat’s response to plaque and oral bacteria. Genetics, immune response, previous periodontal disease, age, and other individual factors may influence how readily inflammation develops or progresses.
This variation is one reason dental health should be evaluated individually rather than assuming that a certain amount of visible tartar corresponds to a specific stage of disease.
Tooth Crowding and Oral Anatomy
The position and spacing of a cat’s teeth can also influence plaque retention.
Crowded, abnormally positioned, or closely spaced teeth may create areas that are more difficult to keep clean. In addition, abnormal tooth positioning can alter how the teeth and surrounding tissues interact.
However, anatomy should be viewed as a contributing factor rather than a universal explanation for gingivitis. A veterinarian can evaluate whether tooth position or other structural abnormalities are contributing to inflammation in an individual cat.
Other Oral and Systemic Conditions
Not every cat with red gums has simple plaque-associated gingivitis.
Other oral conditions can produce inflammation that may initially look similar to gingivitis. These include feline chronic gingivostomatitis, tooth resorption, damaged teeth, periodontal disease, and other inflammatory or infectious processes.
In some cases, systemic health conditions may also influence oral health or alter the veterinarian’s diagnostic approach.
For this reason, persistent gum inflammation should not automatically be diagnosed at home as “gingivitis” based only on appearance. Red gums describe what you can see. They do not necessarily tell you why the inflammation is occurring.

Gingivitis vs Periodontal Disease in Cats
Gingivitis and periodontitis are related, but they are not the same condition. The most important difference is whether inflammation is limited to the gingiva or has progressed into the deeper structures supporting the teeth.
Gingivitis involves inflammation of the gums without periodontal attachment loss. Periodontitis involves inflammation and destruction of periodontal supporting tissues, which can include attachment and bone loss.

Once periodontal attachment and supporting bone have been destroyed, simply removing plaque or brushing the teeth cannot restore those structures.

Periodontal disease also cannot be accurately staged simply by looking at how red the gums appear. A veterinarian may need periodontal probing and dental radiographs to determine what is happening beneath the gumline.

Dental Disease in Cats
Dental disease in cats is a broad term for conditions affecting the teeth, gums, and structures that support the teeth. Gingivitis is one form of dental disease, but it is not the only one. Cats can also develop periodontitis, tooth resorption, fractured teeth, stomatitis, and other oral conditions.
This distinction matters because two cats with red gums or difficulty eating may have very different underlying problems. A veterinary examination, and sometimes an examination under anesthesia with dental radiographs, may be necessary to determine exactly what is happening.
Gingivitis
Gingivitis is inflammation of the gingiva, or gums. In plaque-associated gingivitis, inflammation develops along the gumline without loss of periodontal attachment.
Gingivitis is classified as Stage 1 periodontal disease (PD1) when there is inflammation but no attachment loss. At this stage, the architecture and height of the alveolar bone margin remain normal.
Periodontitis
Periodontitis occurs when inflammation extends beyond the gingiva and affects the tissues supporting the teeth, including the periodontal ligament and alveolar bone.
Unlike gingivitis, periodontitis involves periodontal attachment loss. As the disease advances, supporting bone can be lost and affected teeth may eventually become mobile.
The AVDC divides periodontitis into early, moderate, and advanced stages according to the amount of periodontal attachment loss.
Tooth Resorption
Tooth resorption is the progressive loss of a tooth’s hard tissues. It is a separate dental condition and should not be confused with gingivitis or periodontal disease.
Resorption may affect the crown, root, or both. Importantly, some changes occur beneath the gumline and cannot be fully evaluated by simply looking inside a cat’s mouth. Dental radiographs are important for classifying the type and extent of resorption and helping determine appropriate treatment. The AVDC classifies tooth resorption separately by both stage and radiographic type.
Fractured or Damaged Teeth
Cats can also develop cracked, fractured, or otherwise damaged teeth. A fracture can range from damage limited to the enamel to a deeper fracture involving dentin, the pulp, crown and root.
The seriousness of a fractured tooth therefore depends on which structures are affected. Some fractures can expose the sensitive pulp inside the tooth and require veterinary treatment.
Feline Stomatitis
Feline stomatitis is not simply severe gingivitis. It is a painful inflammatory condition involving the oral mucosa and commonly extends beyond the gingiva into other areas of the mouth.
The AVDC describes feline stomatitis as inflammation of the oral mucosa, often involving the tissues toward the back of the mouth and sometimes additional oral surfaces. It can cause substantial oral pain and changes in eating, grooming, and behavior.
The key distinction is that not every cat with dental disease has gingivitis, and not every inflamed mouth represents uncomplicated plaque-associated gingivitis. Identifying the specific disease is important because treatment differs considerably between these conditions.

Cat Dental Disease Stages
Veterinarians use periodontal disease stages to describe the severity of disease affecting an individual tooth. The American Veterinary Dental College recognizes stages from PD0 through PD4, ranging from a clinically normal periodontium to advanced periodontitis.
One particularly important point is that periodontal staging applies to individual teeth, not necessarily the cat’s entire mouth. A cat can have teeth at different stages of periodontal disease at the same time.
Stage 0: Clinically Normal Periodontium
PD0 means the tooth’s periodontal tissues appear clinically normal.
There is no clinically evident gingival inflammation or periodontitis. This represents periodontal health rather than disease.
Stage 1: Gingivitis
PD1 is gingivitis without periodontal attachment loss.
The gums are inflamed, but the structures supporting the tooth have not yet experienced the attachment loss that defines periodontitis. The alveolar bone margin remains normal.
This distinction is important because gingivitis and periodontitis are not interchangeable diagnoses.
Stage 2: Early Periodontitis
PD2 is early periodontitis with less than 25% periodontal attachment loss.
At this stage, disease has progressed beyond inflammation confined to the gums. Early radiographic evidence of periodontitis may be present, and multirooted teeth may have early furcation involvement.
Stage 3: Moderate Periodontitis
PD3 represents moderate periodontitis with 25% to 50% attachment loss.
Supporting periodontal structures have sustained substantially more damage. The stage may be determined using clinical attachment measurements, dental radiographs, or the degree of furcation involvement in multirooted teeth.
Stage 4: Advanced Periodontitis
PD4 is advanced periodontitis with more than 50% attachment loss.
This is the most advanced AVDC periodontal stage. There has been extensive destruction of the structures supporting the affected tooth, and advanced furcation involvement may also be present.

It is important not to use this chart as an at-home diagnostic tool. You cannot accurately determine a cat’s periodontal stage simply by looking at the color of the gums or amount of visible tartar. Periodontal probing and dental radiographs may reveal attachment and bone loss that cannot be seen above the gumline.
Can Gingivitis Progress to Periodontal Disease?
Yes. Plaque-associated gingivitis can progress to periodontal disease if inflammation persists and begins affecting the deeper tissues that support the teeth. However, gingivitis and periodontitis are not interchangeable diagnoses.
The progression can be understood as:
Plaque accumulation → gingival inflammation → persistent inflammation → periodontal tissue involvement → attachment and bone loss
With gingivitis, inflammation is limited to the gingiva and there is no periodontal attachment loss. At this stage, appropriate treatment may allow the inflamed tissues to return to health.
Periodontitis begins when inflammation extends into the supporting structures of the tooth and periodontal attachment is lost. Depending on severity, this can include destruction of the periodontal ligament and loss of alveolar bone.
This distinction is important because gingivitis can potentially be reversible, while periodontal attachment and bone that have already been lost are not simply restored by brushing or removing visible tartar.
Progression is also not identical in every cat or every tooth. A cat can have healthy teeth, gingivitis, and different stages of periodontitis present in the same mouth. How Is Gingivitis in Cats Diagnosed?
A veterinarian diagnoses gingivitis in cats by examining the gums and determining whether inflammation is limited to the gingiva or is associated with deeper dental or periodontal disease.
Diagnosis may involve several steps.
Medical and Dental History
The veterinarian may ask about changes such as bad breath, drooling, difficulty eating, dropping food, changes in food preference, reduced grooming, or sensitivity around the mouth.
Previous dental procedures and known oral problems can also provide useful context.
Awake Oral Examination
An initial oral examination can reveal visible findings such as:
- red or swollen gums
- plaque and calculus
- bleeding
- damaged or missing teeth
- obvious tooth resorption
- oral inflammation
- masses or other abnormalities
However, an awake examination has important limitations. A conscious cat generally cannot receive the detailed tooth-by-tooth examination necessary to fully assess periodontal structures.
Assessment Under Anesthesia
When a comprehensive dental evaluation is indicated, anesthesia allows the veterinary team to safely examine each tooth and the surrounding tissues.
This makes it possible to evaluate areas that cannot be adequately assessed while the cat is awake.
Periodontal Probing
A periodontal probe is used to evaluate the space around individual teeth and identify abnormal periodontal pockets, gingival recession, attachment loss, and other changes.
These measurements help distinguish gingivitis from periodontitis and determine periodontal disease severity.
Dental Charting
Findings are recorded on a dental chart, creating a tooth-by-tooth record of the cat’s oral health.
The chart may document gingivitis, periodontal measurements, missing or fractured teeth, mobility, tooth resorption, and other abnormalities.
Dental Radiographs
Dental radiographs provide information about structures hidden beneath the gumline.

Why Dental X-Rays Matter
A tooth can look relatively normal above the gumline while significant disease exists underneath it.
Dental X-rays allow veterinarians to evaluate:
- tooth roots
- alveolar bone surrounding the teeth
- periodontal bone loss
- tooth resorption
- retained roots
- unerupted or abnormal teeth
- other changes hidden beneath the gingiva
This is particularly important in cats because tooth resorption frequently involves structures that cannot be adequately assessed by looking at the visible crown alone.
Dental radiographs also help determine the true extent of periodontal disease. The amount of visible tartar does not reliably tell you how much attachment or bone has been lost underneath the gumline.

Cat Gingivitis Treatment
Cat gingivitis treatment depends on the underlying cause, severity of inflammation, and whether disease remains limited to the gingiva or has progressed into deeper periodontal structures.
There is therefore no single treatment that is appropriate for every cat with red or inflamed gums.
Treatment begins with determining why the gums are inflamed. A cat with uncomplicated plaque-associated gingivitis may require a very different treatment plan from a cat with periodontitis, tooth resorption, fractured teeth, or feline chronic gingivostomatitis.
How to Treat Gingivitis in Cats
For plaque-associated gingivitis, treatment generally focuses on controlling the dental plaque responsible for inflammation and addressing any additional disease identified during the dental evaluation.
Depending on the individual cat, veterinary treatment may involve:
- a comprehensive oral examination
- professional dental cleaning
- periodontal probing and charting
- dental radiographs
- treatment or extraction of diseased teeth when necessary
- appropriate pain management
- an individualized home dental-care plan after treatment
Importantly, visible tartar should not be the only focus. The goal is to identify and treat disease affecting the entire tooth and its supporting structures.
Once the mouth is healthy enough for home care, regular plaque control can help reduce future accumulation. The appropriate routine depends on the cat and the veterinarian’s findings.

Treatment of Diseased Teeth
Cleaning alone cannot correct every cause of gingivitis or dental pain.
If dental radiographs and examination reveal advanced periodontal disease, tooth resorption, fractures, or other significant tooth pathology, affected teeth may require additional treatment. In some cases, extraction is the appropriate treatment for a painful or structurally compromised tooth.
Treatment decisions are made tooth by tooth based on the diagnosis and severity of disease.
This is another reason a visibly cleaner mouth after scaling does not necessarily mean that all dental disease has been addressed.
Pain Management and Other Treatment
Dental and oral disease can be painful, so pain management may form an important part of treatment. The medications and approach used depend on the procedure performed, the severity of disease, and the individual cat’s health.
Some cats also require treatment for conditions beyond routine plaque-associated gingivitis. For example, feline chronic gingivostomatitis, significant tooth resorption, or other oral diseases require treatment plans specific to those diagnoses.
Antibiotics are also not a substitute for appropriate dental treatment. Whether medication is indicated depends on the individual clinical situation and should be determined by the veterinarian.
Ultimately, successful treatment starts with an accurate diagnosis. “Red gums” are a clinical finding, not a complete diagnosis, and determining what is causing the inflammation is what allows the veterinarian to choose appropriate treatment.
Can Cat Gingivitis Be Cured?
Gingivitis in cats can often be reversed when the underlying cause is identified and appropriately treated, particularly before periodontal attachment or bone loss has occurred. However, whether an individual cat’s gingivitis resolves depends on what is causing the inflammation.
This is where the distinction between gingivitis and periodontitis becomes especially important.
With gingivitis, inflammation is confined to the gums and there is no periodontal attachment loss. When plaque and other contributing factors are appropriately addressed, the gingival tissue may return to health.
